Implants are devices designated to replace the natural patient’s teeth. Usually there are one-piece or two-piece implants..
Two-piece implantsusually consist of two separate parts i.e. an element placed in the jawbone, often having a thread resembling a kind of screw and an implant abutment – a part resembling a trimmed tooth crown, which is screwed onto the intrabony part. A prosthetic crown of various type (temporary or permanent, metal-free porcelain, porcelain bonded to steel, gold, titanium etc.) is placed on it.
One-piece implants are such implants in which an implant abutment and the intarbony part are made as a whole (they are one element) and they are at once placed into the patient’s jaw. Similarly to two-piece implants, a prosthetic crown is fitted on the implant abutment. |

Dental implants are a solution:
1) When a single or a few teeth are missing and we decide to replace the missing tooth with an implant.
2) When a few or many teeth, or even all teeth are missing, and the patient wants to have a permanent replacement (not removable from mouth) – so called implant bridge is made on several implants similarly to a bridge on natural teeth.
3) When many or all teeth are missing and the patient wants to have a more comfortable prosthesis, then we need 2 or 4 implants to considerably improve e.g. the support of the lower full dentures and make the upper dentures without the palatal base. In this case implants often look slightly different from what was mentioned in the beginning. Namely, the intrabony part instead of the implant abutment and the crown we place precise fastenings for prostheses of a different type e.g. ball-shaped studs, magnets, special bars etc.
These solutions often significantly enhance the comfort of patients who still have big problems with eating, speech, smiling due to poor denture stability and also people who do not tolerate the palatal plate and have a strong inclination to vomit, have a changed speech or suffer from taste impairment.

| Preparation for the procedure |
First of all a precise diagnosis is necessary (a prosthetic plan, a general examination, a detailed analysis of the bone amount and bone quality in the area where the implant will be placed, hygiene evaluation). Usually we invite a patient for two visits: the first: a general one, when we determine the needs and wishes of the patient and our alternative ways to achieve his/her goals. We estimate also individual costs. If the patient is interested, we recommend the following examinations: a general blood and urine test, a pantomographic image and/or a computerized jaw tomography. We also conduct several examinations at the dentist’s office (assessment of diagnostic models, thickness of mucous membrane, we take impressions etc). We can also develop a dental implant treatment plan (sometimes it includes procedures of a pre-implantology surgery).

Typically, the procedure is done only with the local anaesthetic. If we chose the two-piece implants, after we have accurately drilled a hole in the jawbone we place an intrabony part of an implant into the bone. And next we stitch the gums back in place for a recovery time (3 to 6 months). After the healing period it is necessary to perform a procedure of exposing the intrabony part and place a healing and encouraging the gum growth cap on it for about two weeks. Then we replace the cap with the implant abutment and we make the prosthetic part.
In case of one-piece implants, the implant abutment is visible in the oral cavity and we proceed to placing a temporary crown on it at once (the implant abutment may need trimming, then we put a rubber dam and we trim it at once).
In case of immediate implants indicated for immediate implantation upon extraction, we prepare the alveolus for implantation. Most often we additionally use a biomaterial (stimulating bone growth). We insert an implant at once and we place a crown on it. This means that the patient has a new tooth upon leaving the dentist’s office.. |
